Sports physical for all middle school and high school students who are planning on participating in athletics during the 2018-2019 school year will be done free of charge on Monday, June 18, beginning at 4:00PM at the Hematite Health Clinic at Ishpeming High School.  All students must have a Hematite Health Clinic consent form signed by a parent and also a MHSAA sports physical form signed by a parent before having the physical exam.  Links to both forms are below. 

In addition to the sports physicals, impact testing for baseline concussion screening will be given to incoming freshman and junior students.  There is also no cost for the impact testing. 

All students are asked to enter the school by using the Fourth Street Old Gym doors and report to the cafeteria prior to their exam.

Any student unable to attend on June 18 can schedule an appointment with the Hematite Health Clinic anytime during their summer hours which are from 7:30AM – 3:00PM Monday – Thursday and 7:30AM – 11:30AM on Friday.  Call to schedule an appointment at 204-2620 but walk-ins are also welcome. 

Remember, a completed sports physical is needed before the first day of any fall sports practices.
