Will there be a charge for visits?

Insurance will be billed for most visits, but no child will be turned away for not having insurance.

How can students be seen in the school based clinic?

Students must have a consent form signed by their parent/guardian to receive care, if they are under age of 18.

How will the clinic staff work with your primary care providers?

We encourage continuity of care and will communicate with the student’s primary care provider.  The signature on the parent/guardian consent form allows the Hematite Health Clinic and the child’s primary care physician to exchange health care information for the purpose of continuity and coordination of care according to State and Federal laws.  Clients have the right to decline sharing this information with their primary care provider.
